2013 Guidelines for Cholesterol Control

Fraud Waste & Abuse Prevention

Evidence Based Medicine

Guidelines created by Evidence Based Medicine Committee are provided below.

Clinical expertise, patient values and education, and best evidence are combined to become (EBM) Evidence Based Medicine. Clinical expertise is provided by the clinician's cumulated experience, education and clinical skills. The patient provides his or her own personal and unique concerns, expectations, education, and values. The best evidence is usually found in clinically relevant research that has been conducted using sound methodology.

Please note, these guidelines are not 'suggestions' but rather 'guidelines' to ensure that our QIP patients are receiving the appropriate medications and follow up.

Patient Education Materials on major topics such as Angina, Living with Congestive Heart Failure, Medication Safety, Chronic Obstructive Pulmonary Disease (COPD), Preventing Trips and Falls, Preventing Deep Vein Thrombosis (DVT), Type 2 Diabetes, Urinary Tract Infections, and Smoking Cessation are under the Paitient Education section of this website.

(COPD) Chronic Obstructive Pulmonary Disease

Here are a few documents and links outlining the QIP COPD clinical guidelines. These guidelines have been drawn from current literature on the care of stable COPD patients.

  • tutorial The clinical guidelines (PDF Download) Please note the need to document FEV1 or FEV1/FVC in the chart. The measurement does not have to be done by you, but can be taken from pulmonologist office notes or data. Many of the treatment recommendations are based on the degree of pulmonary dysfunction, so we, as a committee believe this should be documented every two years.

  • tutorial Patient & Zone Teaching Handout (PDF Download) Again, patients will generally do better if they have better understanding of their condition it's sign and symptoms and when to contact your office. This handout should be reviewed at least yearly with your COPD Paitient.

  • tutorial The "Chronic Obstructive Pulmonary Disease Program" (Learn More) As a committee, we strongly believe that patient education is paramount to success in the treatment of COPD. We feel that educating patients about their disease process and how to manage changes in their clinical condition is essential in optimizing care and reducing hospitalizations.

(CHF) Congestive Heart Failure

Here are a few documents and links outlining the QIP CHF clinical guidelines. These guidelines have been drawn from current literature on the care of stable CHF patients.

  • tutorial The clinical guidelines (PDF Download) The guidelines were taken from data gathered from a recent issue of the Prescriber's Letter. While you may not find the data new, it will give the organization a framework for ensuring we treat our patients with the most up to date regimens recommended. As with our other guidelines, we will be doing audits of charts over time to ensure that guidelines are being followed.

  • tutorial A patient information sheet on Pneumonia (PDF Download) You or your Medical Assistant should review and notate for the patient to take home
